Breast Revision Surgery: Signs It Might Be Time for a Redo

Deciding to update the results of a past breast surgery is a personal choice, but one that is more common than many people realize. Dr. Karol Gutowski is an award-winning surgeon who specializes in breast revisions, working with patients so their implants fit their body and their lifestyle. This surgery is a way to correct problems with previous implants or simply adjust a result so it aligns more with your preferences.

What Does Breast Revision Surgery Offer?

Breast revision surgery updates or improves on the results of a previous breast surgery. The procedure can correct issues like implant rupture, capsular contracture, rippling or shifting implants. Implants can be exchanged for a different shape, size or material, or they can be removed and combined with a lift to restore their original size and position.

Physical Signs You May Need a Breast Revision

Changes in Implant Feel or Firmness

Sometimes after a breast augmentation, the breast may begin to feel different. If the implant becomes noticeably firm or tight, this can be a sign of tightening scar tissue, a condition called capsular contracture. You may also notice the breast lifting higher than it used to. Each of these changes can be gradual, so be sure to discuss them with your surgeon as soon as you notice them.

Visible Rippling or Shifting

Rippling can occur with some breast implants along the sides of the breast near the armpit, or at the bottom near the crease. This is more common in people with thinner skin or less breast tissue, and happens most often with saline implants. Implants can also drop more than intended, rotate or settle past the natural crease. Any time you see the shape changing, a revision can help restore the breasts.

New Pain or Asymmetry

Discomfort or pain in the breasts is always something to pay attention to. Sensitivity is normal early on in the recovery process, but once the incisions have healed and swelling has settled, you should not have any new or ongoing pain. If one breast starts to look different from the other in size, height or shape, bring this up with your surgeon as well.

Breast Implant Rupture

Both saline and silicone breast implants can rupture. With saline implants, you will know right away because one breast will deflate. Silicone ruptures are harder to detect and are typically only diagnosed through imaging appointments, which should be scheduled regularly. Both are reasons for a revision.

Other Signs That It Is Time for Breast Revision

Sometimes patients come in for aesthetic reasons, even if their implants are still in good condition. If you are considering revision, it may be because your goals or life circumstances have changed since your first surgery.

Changes in Aesthetic Preference

The look that you loved years ago may no longer match the style you want today. Perhaps you want something more subtle, or you may be ready for more volume. In either case, revision lets you update your implants to match your goals.

Weight Fluctuations

When you gain or lose weight, this can change the proportions of your body, and these weight changes can affect how your breasts look or feel.  Weight loss can lead to sagging in the breasts as skin elasticity changes, and there is less volume in the breasts. If your body has changed since your surgery, a revision can help your breasts to be more proportional.

Pregnancy

Pregnancy and breastfeeding often impact the breasts in lasting ways, with increases and decreases in volume that can leave them looking more stretched. Many patients choose to wait until they are done having children to have breast implants, but this is not always the case. If pregnancy and childbirth have altered the results of a previous surgery, revision will correct the shape and size of the breasts.

A Desire for Different Implant Options

Implant technology has advanced significantly over the past decade, and you may be interested in exploring what type of options are available today. Some patients want to move from saline to silicone for a softer feel, while others want a different profile or size.

Your Breast Revision Options

Implant Exchange for a New Size or Style

An implant exchange swaps your current implants for new ones. This is a choice for patients who want to adjust their size, change the shape or profile or switch materials. It is also a good solution when an implant has ruptured, resolving the underlying issue.

Implant Removal With or Without a Lift

Some patients decide they would rather return to a more natural breast size. In this type of revision, the implants are removed, and the skin can be tightened and reshaped so the breast looks natural. Adding a lift restores a youthful position when the skin has stretched.

Capsulectomy to Remove Scar Tissue

When capsular contracture develops, the hardened scar tissue around the implant is surgically removed in a procedure called a capsulectomy. This relieves the tightness and discomfort contracture can cause. Removing the capsule also creates a healthy foundation for a new implant if you choose to have one placed.

Revision Reinforced With Surgical Mesh

For more complex cases, Dr. Gutowski may reinforce the repair with a surgical support material such as Strattice. This mesh acts like an internal scaffold, giving the implant added stability and helping it stay in the ideal position for long-lasting, natural-looking results.

Misconceptions About Breast Revision Surgery

Revisions Can Only Be Done When Something Went Wrong

While revision can absolutely correct complications, many patients choose it simply because their preferences have changed. Wanting a new size or an updated look is a perfectly valid reason on its own.

Revision Surgery Takes Longer to Recover From

Many people assume a second surgery means a longer recovery, but the opposite is often true. Because the implant pocket already exists, healing can feel more familiar and comfortable the second time around, though your exact timeline depends on the details of your procedure and its complexity.

You Must Return to Your Original Surgeon

You are always free to choose a new surgeon for your revision. What matters most is finding a board-certified plastic surgeon experienced in revision work. Dr. Gutowski regularly helps patients who had their first procedure elsewhere and want a fresh, expert perspective.

New Implants Don’t Need Monitoring

Even after a successful revision with newer, more advanced implants, ongoing monitoring will still be an important part of the process. Silicone implants can still develop a silent rupture that will need to be addressed. Routine imaging is the best way to keep an eye on your implants and enjoy peace of mind.

How Long Do Breast Implants Last?

While breast implants don’t have an expiration date, they are not made to last indefinitely. As they get older, the chance of wear increases. Research published in Plastic and Reconstructive Surgery reports a cumulative rupture rate that climbs the longer the implants stay in place. In general, you can expect implants to last ten years or longer.

About Capsular Contracture

After any implant is placed, your body naturally forms a thin layer of scar tissue around it called a capsule. This is a normal, healthy response. In some cases, though, that capsule tightens and thickens, squeezing the implant and creating firmness or a change in shape.

How often this happens can vary. A meta-analysis published in Plastic and Reconstructive Surgery found that implant type, placement and incision location can all influence the likelihood of contracture developing. Surgeons often grade contracture using the Baker scale, which ranges from soft and natural to firm and visibly altered. If you notice tightening, an evaluation can determine the grade and guide the most comfortable path forward.

Candidacy for Breast Revision Surgery

You Are Healthy Enough for a Second Surgery

Good candidates are in stable overall health and able to heal well. Maintaining a steady weight, avoiding smoking or being willing to quit, and keeping any medical conditions well-managed are all good signs that you will respond well to surgery. Dr. Gutowski always makes safety a priority as he reviews your history.

Enough Time Has Passed Since Your Last Procedure

Except in the event of serious complications, it is important to allow your body to fully heal from your original surgery before pursuing a revision. A window of six to twelve months is common, unless an urgent concern like a rupture calls for earlier attention.

You Understand Your Options

The best candidates are informed and clear about what they want to change. When you understand the possibilities available and have realistic expectations, you can make a good decision. Dr. Gutowski walks you through every option so you feel supported.
